What type of identification do I need to bring?
To issue a new or replacement Blue Alcohol Card (BAC), we need to verify your identity with a valid picture ID. The requirements are simple:
- Primary ID: If you have a valid U.S. Driver’s License, DMV-issued ID Card, or a U.S. Passport, that’s all you need.
- Secondary IDs: If you don’t have one of the above, you must present two forms of identification from the valid identification list below. At least one of the two must contain your photo.
How can I check if my Blue Alcohol Card (BAC) is expired?
Blue Alcohol Cards (BAC) are valid for four years from the date they are issued. If you’re unsure about your card’s expiration date, please contact our office. Our staff can look up your status in our database.
Valid Identification List
All applicants for a Blue Alcohol Card (BAC) must provide valid, original identification documents (photocopies are not accepted).
Primary Identification (One Required)
- U.S. Driver’s License
- DMV-issued ID Card
- U.S. Passport
Secondary Identification (Two Required if No Primary ID)
If you do not have one of the primary forms of ID listed above, you must present two from this list. One must contain your photo.
- Driver license from another country
- Identification card issued by a foreign consulate (matricula consular)
- Social Security Card
- Utility bill in your name and address (electricity, water, gas)
- Local work ID (industry related)
- Bankcard ID (with photo)
- Sheriff’s Card
- Certified United States birth certificate
- Certified foreign birth certificate
- Certificate of Citizenship or Naturalization
- Consular report of birth abroad
- Permanent or temporary Resident Card
- Resident Alien Card or 1-551 Receipt
- Employment Authorization Card
- United States Military Identification or Military Report of Separation
- Local school ID with birth certificate (for minors under 18)
